* Self-employed: most recent tax return showing income
* Government/retirement income: year-end statement or benefit/award letter

Emergency Contact
* Must be someone who does not live in the home

Liability Insurance (this must be done when lease is signed)
* Most properties require $100,000 landlord liability insurance
* Upload declaration page showing:
* McKimmey Associates Realtors listed as an interested party
* Insurance agent's name and policy number
* Coverage must remain active throughout the lease

Vehicles
* List all vehicles to be kept at the property, including license plate numbers

Occupants
* List all individuals who will reside in the home, including dependents who stay regularly
* Include name, date of birth, and relationship
* Each occupant 18 years or older must submit a separate application

Service Animals & ESAs
* Service animals and ESAs are not considered pets
* Disclosure and proper documentation are required in accordance with the Fair Housing Act
* Misrepresenting an ESA as a Service animal in Arkansas may result in fines up to $2500.
