* Insurance agent's name and policy number
* Coverage must remain active throughout the lease
Vehicles
* List all vehicles to be kept at the property, including license plate numbers
Occupants
* List all individuals who will reside in the home, including dependents who stay regularly
* Include name, date of birth, and relationship
* Each occupant 18 years or older must submit a separate application
Service Animals & ESAs
* Service animals and ESAs are not considered pets
* Disclosure and proper documentation are required in accordance with the Fair Housing Act
